COMFA IT – New User Equipment Request

Please follow the instructions below carefully to ensure the successful setup of equipment and system access.

When you click the form button at the bottom of the page, the PDF will open in your web browser.
⚠️ Please make sure to download the form to your computer before filling it out. Do not complete it within the browser, as your information may not save correctly.

All fields marked in red are required.
Make sure to complete all relevant fields with accurate information.

🚫 Do not enter placeholder or fabricated data.
This information is used to:

  • Configure the new equipment
  • Register devices on the UF network
  • Add records to UF’s asset management system

⚠️ Important: UF Employment or Student Status

One of the fields will ask:

“Is the user a current or former UF employee or student?”

If you select “Yes” (i.e., the user is currently employed or enrolled at UF), please note:

  • The user must not log in to a COMFA machine before their official start date.
  • Logging in early may lead to access issues after their start date.
  • If early login is absolutely necessary, you must contact COMFA IT Support first.

📄 Submit the Form

  1. Complete all required fields and any additional applicable information.
  2. Click the “Submit to IT” button at the bottom of the form.
  3. You will be prompted to save the form:
    • Save it to any location.
    • Name the file using the new user’s GatorLink ID.
  4. After saving, you will be prompted to send an email.

📧 Sending to COMFA IT

  1. Choose “Default email application (Microsoft Outlook)” when prompted.
  2. Click Continue.
  3. An email window will appear with the completed form attached.
  4. Click Send.

✅ You will receive a confirmation email indicating that a ticket has been opened with COMFA IT Support.
